Person-Centered Service Planning

Therap’s Person-Centered Service Planning module provides a platform to facilitate discussions and help in the decision making process when creating a person-centered service plan. Agencies can use this tool to plan, organize and implement the individualized service plans for each individual in a HIPAA compliant manner. This tool helps in developing detailed Person Centric Service Plans, determining goals, objectives & scoring method and generating reports for review & analysis.

Employment History

The Employment / Volunteering History form provides a comprehensive summary of the services in one place. It details assessments and accomplishments the individual has made throughout their work experiences. This module helps to create a job bank of employers and record contact interactions, maintain an individual’s job history, keep track of referrals, milestones, career development plan and more. These records can be used on applications to help them find future jobs.

Nursing/ Progress Notes

T-Logs are an alternative to your logbooks and the daily notes that you share with other staff members at your agency. You may write nursing/ progress notes for specific individuals or for a particular program. These logs are automatically shared with co-workers based on their caseload of individuals. Updates made to T-Logs can be seen from Update History.
