The Individual Demographic Form (IDF) is designed to help users maintain essential information such as religion, admission date, program enrollment date, etc. for individuals, along with their basic identification details such as name, date of birth, physical features, etc. Users are able to attach two photographs and scanned documents and store them in the form.

When entering medical information in the IDF, users can easily select the medical diagnoses that apply to an individual. Users can also look up the individual’s physician, dentist, hospital and pharmacy from the medical contact list. Users also have the option to Resolve a Diagnosis and document the date it was resolved and by whom.

Information on the IDF can be updated by users who have been assigned appropriate privileges.

Demographic Report

Demographic report is a useful tool for agencies to get all the relevant demographic statistics by language, gender, mobility, communication modality, country of origin, top 10 diagnoses [ICD-10/ICD-9/ DSM-5/Other] and others. Each agency can generate a demographic report, which is based on the information entered in the Individual Data forms. The report also shows counts of individuals enrolled in different programs and supervision schemes.
